Schedule:

• Week 1 – Days (8 AM - 4:30 PM) - Home Visits
• Week 2 – Nights (4:30 PM - 8 AM) - Work from home (making and taking calls)

Education & Certifications Required:

• Required Certifications: BLS
• Education Modules: Per hospice protocol

Unit Information:

• Patient Ratios: Manage 14-18 patients (based on acuity) with an expectation of 5-6 visits per day. Minimum productivity is 25 units per week.
• Equipment: Assigned an agency laptop, iPhone with case and chargers, nursing supply bin, nursing bag with vital signs equipment, and an agency bag. All equipment must be returned upon departure.
• EMR: EPIC

Hiring Profile:

• Must Have Skills/Experience: Proficiency in EPIC
• Preferred Skillset/Experience: Experience with PICC lines, dressing changes, tracheostomy care, port access, ostomies, chest tubes, Pleurex drains, catheters, lab draws, and urinalysis.
• Type of Patients: Adult medical surgical, oncology, cardiac, neurological, respiratory, wound care, GI, and GU patients.
• Personality Fit: Collaborative, team-oriented, strong communicator, open to feedback to improve practice or workflow.

Dress Code:

• Scrub Color: Business casual or scrubs (any color)
• Unacceptable Attire: No jeans, open-toed shoes, or clothing with holes

Miscellaneous Information:

• Must be able to effectively case manage and self-schedule patients.
• Involves the use of in-home infusion pumps. No ventilators or invasive monitoring required.
