Job Description

The Home Care Registered Nurse plays a vital role in providing high-quality patient care in accordance with evidence-based practices, state and federal regulations, and agency policies. Key responsibilities include:

• Completing OASIS documentation in a timely manner while adhering to agency policies, procedures, and regulatory standards.
• Assessing patient and caregiver needs using the nursing process to deliver quality care to assigned patients.
• Developing and communicating a realistic plan of care that involves patients and caregivers in the planning process.
• Serving as a Case Manager for assigned patients, actively managing caseloads while meeting agency productivity standards.
• Implementing medical plans of care, nursing interventions, and procedures to ensure safe and effective care delivery.
• Staying current on advanced nursing skills, including IV therapy, wound VAC, complex wound care, and infection control standards.
• Collaborating with LPNs to ensure care plans are executed effectively and delegating tasks appropriately.
• Evaluating the effectiveness of the plan of care and making adjustments as needed based on patient responses.
• Maintaining accurate and up-to-date documentation of nursing care to support ongoing quality care collaboration.
• Following agency protocols related to documentation and visit management.
• Providing education to patients and caregivers on medications, treatment options, and care procedures.
• Identifying and documenting interdisciplinary needs and coordinating care with other team members.
• Accurately completing agency, state, and federal forms and ensuring they are available during patient visits.
• Efficiently managing visit schedules and communicating any necessary information to the scheduling team.
• Assisting patients in accessing Home Care Aide services and supervising caregiver activities as needed.
• Reporting patient incidents such as infections or injuries to the Quality Improvement Specialist in a timely manner.
• Coordinating safe patient discharges or transfers, ensuring all paperwork and information are processed correctly.
• Facilitating seamless communication with physicians, providers, and team members to uphold recommended care plans.
• Participating in peer review audits, providing and receiving constructive feedback professionally.
• Scheduling and leading case conferences to improve team communication and care coordination.
• Advocating for patient care needs to ensure sensitive and quality care that meets individual and family preferences.

Additional Requirements

Availability to cover holidays and weekends is necessary.
