Description

Registered Nurse (RN)

Join our team as a Home Health Registered Nurse! We are seeking an experienced RN with a passion for providing compassionate care in a home health setting. The ideal candidate will have OASIS experience and a background in home health and hospice care.

Shift Information

Friday, Saturday, Sunday, and Monday from 8 am to 6 pm
Schedule flexibility based on patient needs
1 weekend a month scheduled, followed by 2 days off

Responsibilities

Conduct home visits to assess patient needs and develop care plans.
Complete OASIS assessments accurately and timely.
Provide skilled nursing care and support to patients in their homes.
Manage changing schedules and adapt to patient needs.
Maintain accurate documentation using the EMR system HealthWyse (Careficient).
Participate in on-call duties on an emergency basis as needed.

Requirements

Minimum of 2 years of nursing experience required.
OASIS (Outcome and Assessment Information Set) experience is mandatory.
Home Health and Hospice experience is required.
Must possess a valid driver's license in good standing.
BLS (Basic Life Support) certification is required.
Ability to travel between patient homes for visits.
Comfortable working in an outpatient care environment.
Background check will be conducted.
