Description
This position may qualify for a sign-on bonus.
Under general administrative direction, coordinates referrals from physicians and health care facilities for patients; provides a significant level of patient education related to their illness and planned treatment. Responsible for guiding the patients with orthopedic and cardiac DRGs through the complex care continuum. Assures patients receive care that consistently follows evidence-based standards and is in accordance with regulatory standards as defined by the regulatory agencies. Completes established competencies for the position within designated introductory period. Other related duties as assigned.
MINIMUM EDUCATION: Associates Degree of Science in Nursing
PREFERRED EDUCATION: Bachelors of Science in Nursing
MINIMUM EXPERIENCE: 3 years clinical experience for patient population in specialty area
PREFERRED EXPERIENCE: Prior patient teaching
REQUIRED CERTIFICATIONS/LICENSURE: Possession of current Texas State license for Registered Nurse.
PREFERRED CERTIFICATIONS/LICENSURE: Specialty Certification
REQUIRED COURSES/ COMPLETIONS (e.g., CPR): Must have active healthcare provider Basic Life
Support on hire (ARC or AHA); however, must have AHA Healthcare Provider BLS within 60 days of hire.
