🌟 Home Health Registered Nurse (RN) – Travel Contract

📍 Bennington,Vermont
📅 13-Week Contract
⏰ 37 Hours per Week
☀️ Schedule: Monday–Friday | 8:00 AM–4:00 PM
📞 Some Weekends & On-Call Required
🏡 Setting: Home Health

✅ Position Highlights

Provide skilled nursing care to patients in their homes.

Complete OASIS assessments and documentation accurately and on time.

Develop, implement, and evaluate individualized patient care plans.

Manage assigned patient caseloads and coordinate home visits.

Perform IV therapy, wound VAC management, and complex wound care.

Supervise and collaborate with LPNs and other healthcare professionals.

Coordinate patient education, discharges, transfers, and interdisciplinary care.

Hospice coverage required while on-call.

EMR: McKesson

📋 Requirements

Minimum 3 years of Home Health nursing experience required

Home Health nursing experience.

Knowledge of OASIS documentation and home health regulations.

Experience with IV therapy, wound VAC, and complex wound care.

Strong patient assessment, case management, and documentation skills.

Ability to work independently and coordinate multidisciplinary care.
