You will be responsible for delivering high-quality care to patients in their homes, ensuring their rehabilitation goals are met.

Key Responsibilities:

• Provide physical therapy services to home health patients.
• Manage a caseload of 5-6 patients per day, with a focus on achieving productivity standards.
• Document patient progress and maintain timely communication.
• Utilize provided documentation tools effectively.
• Participate in patient care planning and collaborate with other healthcare professionals.

Requirements:

• 1 year of home health experience preferred.
• Current California Physical Therapist license.
• BLS certification required.
• Must have reliable transportation; occasional maximum travel radius of 50+ miles.

• Compliance process takes about 2 weeks.
