Join a leading healthcare provider as a Physical Therapist specializing in Rehabilitation in Webster, NY. This contract position offers an opportunity to provide case management and direct patient care within a collaborative team environment. The qualified therapist will administer comprehensive physical therapy modalities in the home setting, assisting physicians in evaluating levels of function and developing personalized care plans.

Responsibilities:

• Participate in and ensure that Care Delivery Services align with the agency's vision, mission, values, and strategic goals while meeting customer needs.
• Promote effective working relationships and collaborate with team members to achieve organizational goals.
• Utilize resources efficiently to enhance the value of services provided to the community.
• Maintain respectful communication with patients, families, and staff.
• Develop and revise treatment and discharge plans in conjunction with patients, families, and the care delivery team.
• Provide supervision to Physical Therapy Assistants (PTAs) to ensure health, safety, and compliance with care plans.
• Consult with and educate patients and families regarding disease processes, self-care techniques, and preventive strategies.
• Evaluate patient progress and adjust therapy programs as necessary.
• Participate in patient conferences to discuss care plans and ensure coordinated treatment.
• Complete all clinical records to comply with state and federal regulations.
• Maintain knowledge of available community-based services and resources.

Qualifications:

• B.S., M.S., or D.P.T. degree in Physical Therapy
• BLS certification
• Current New York State License in Physical Therapy
• Minimum of one year of experience in Long Term Care/Home Care setting as a Physical Therapist
• Recent Home Care experience within the last year
• Strong verbal and written communication skills
• Ability to work effectively with diverse populations
• Ability to accept and utilize supervision
