Description
Seeking RN – Home Health/Hospice for a 25-week contract in Vermont.
Start Date: 9/21/2026 | Shift: Days | Monday–Friday | 8:00a–5:00p
Experience: 2+ years | Cert: BLS | License: Must be currently active
EMR: HealthWyse (Careficient)
Requirements: OASIS experience required | Home Health & Hospice experience | Patient-to-patient home visits | Valid driver’s license in good standing.
Schedule: Some weekend coverage and emergency call required. Must be flexible with schedule changes.
Holiday: Required | Setting: Outpatient/Home Health
