Job Summary (List Format):

- Collect and submit complete work history/resume.
- Complete a skills checklist within 1 year of submission date.
- Provide license verification for required role.
- Submit copies of all required certifications.
- Provide at least one supervisory reference at submission.
- Supply proof of taxable address (acceptable documents include driver’s license, utility bill, lease/rental agreement, mortgage statement, insurance statement, voter registration, bank statement, auto insurance, or tax return).
- Certification requirements as specified for the role.
- Must hold a valid Wisconsin state license.
- Compact state license is allowed.

Med Ninjas Job ID #20571. Pay package is based on 12 hour shifts and 36 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
