Job Summary (List Format): Social Worker – MiraVista, Holyoke, Massachusetts

- Provide direct and indirect clinical social work services to patients, families, and legal guardians in an inpatient behavioral health or acute care setting.
- Serve as an active member of a multidisciplinary treatment team, collaborating with physicians, nurse practitioners, and direct care staff.
- Conduct biopsychosocial assessments and participate in multidisciplinary treatment planning and case management.
- Deliver individual and family therapy, systems interventions, and develop comprehensive discharge and aftercare plans.
- Assist with insurance utilization review and act as a liaison between patients, their families/guardians, the hospital, and community agencies for care coordination.
- Contribute to treatment plans by developing, implementing, and documenting appropriate goals and interventions tailored to patient needs.
- Utilize advanced knowledge of clinical behavioral health treatment and family systems modalities for psychiatric illness and conduct risk and safety assessments.
- Maintain accurate records using Electronic Health Record systems; experience with Medsphere/Vista preferred.
- Require LCSW, LICSW, or LMHC licensure (or eligibility) and a Clinical Master’s Degree; CANS certification required within 30 days of hire.
- Prior experience in inpatient behavioral health, short-term treatment, or acute care settings preferred.
- Bilingual English/Spanish skills preferred.
- Full-time, day shift position with no weekend hours.
