Location: Eastern Pennsylvania

Compensation: Details provided to qualified applicants

Schedule: 8-hour day shift, 8:00 AM – 4:30 PM, Monday through Friday with no weekend requirements. Approximately 5 patient visits per day supporting an average daily census of 45 patients.

Highlights:

• Home-based hospice nursing position serving patients throughout the community with terminal diagnoses
• Comprehensive case mix including routine visits, start-of-care assessments, transfers, and significant change in condition evaluations
• Manageable patient volume with approximately 5 visits per day in a supportive interdisciplinary environment
• Epic charting system preferred for streamlined documentation
• Work with adult and geriatric patient populations providing compassionate end-of-life care
• Strong clinical support including interpretation services, social services, and pharmacy

Requirements:

• Active Pennsylvania Registered Nurse license
• Minimum 1 year of RN/Home Health experience required
• Hospice case management and admission assessment experience required
• Community hospital experience preferred
• CHPN certification preferred
• Strong palliative symptom and pain management experience required
• Proficiency in wound care, pressure ulcer staging and management
• Experience with feeding tubes, G-tubes, NG tubes, ostomy and urinary catheter management
• Competency in medication administration including IM, IV, and subcutaneous routes
• Peripheral and central line care with phlebotomy skills
• Respiratory care experience including chest tube/PleurX management, nebulizer education, oxygen administration, and tracheostomy care
• Post-death protocol experience required
• Ability to coordinate services and complete comprehensive clinical assessment documentation
• Comfortable performing routine visits, start-of-care visits, transfers, and assessments for significant changes in condition
• Flu vaccination required (medical/religious exemptions only)
• Reliable transportation for home visits throughout the service area
