Job Description

The Physical Therapist will effectively screen, evaluate, and rehabilitate patients with physical functioning disorders and/or wound care. The role involves developing appropriate care plans in line with regulatory and clinical practice standards, utilizing standardized assessments and evidence-based practices to support clinical interventions.

Responsibilities

• Administer various rehabilitation procedures such as manual techniques, ambulation, therapeutic exercises, modalities, and wound care.
• Assess patient progress and adjust treatments in the clinical record according to regulatory and clinical requirements.
• Evaluate treatment outcomes for effectiveness and modify services as needed, including making referrals.
• Provide consultation and counseling to patients, families, caregivers, and other service providers concerning physical disorders.
• Communicate effectively with supervisors and healthcare team members regarding patient progress and treatment plans.
• Document all required information in the electronic documentation system and maintain patient records according to company policy.
• Participate in meetings such as patient care conferences and rehabilitation meetings, and in required trainings and in-services.
• Supervise and support physical therapist assistants, rehabilitation aides, and/or students.

Required Experience

• 1 year of Physical Therapy experience required.
• Experience in Long Term Care or Rehabilitation preferred.
• Level II experience for new graduates is acceptable.

Certifications/Licensure

• Active Physical Therapist license in Oklahoma is required.
