Job Description
The Occupational Therapist will provide care in the home health setting, delivering individualized therapy services to patients in various locations, including Oxford, Nottingham, West Grove, Lincoln University, Landenberg, Kennett Square, and Avondale. The role involves working closely with patients to assess their needs, develop treatment plans, and facilitate rehabilitation, ensuring adherence to established protocols.
Responsibilities
- Evaluate patient needs and develop tailored treatment plans.
- Implement therapeutic interventions in the home health environment.
- Document patient progress and adjust treatment plans as needed.
- Collaborate with other healthcare professionals to optimize patient outcomes.
- Ensure compliance with all documentation and reporting requirements.
Required Experience
- Minimum of 2 years of experience as an Occupational Therapist.
- Previous home health experience required.
- Experience with EPIC documentation system is mandatory.
Certifications/Licensure
- Current Occupational Therapist license in the state of Pennsylvania.
- CPR certification is preferred.
