Shift: 8hr Day Shift (40) Shift Information: 8am-5pm field work, Mon-Friday, Holidays adjust by covering weekend or unpaid day off,
Clinical Interviewing- No On Call, Call Back Requirements: Mon-Friday, Weekends available with take back day during week. Holiday off with working weekend or unpaid holiday off Weekend Rotation: Every 5th week with weekdays off as take back day Holiday Requirements: None - can work weekend before/after with holiday off to make 40 hours or take unpaid holiday off Floating Requirements: South East B primary (60620, 60628, 60643) Float to South East A (60617, 60619)

What You'll Do

• Provide comprehensive physical therapy assessments and develop individualized care plans


• Deliver in-home therapy focused on improving mobility, safety, and independence


• Serve as a case manager, coordinating care with physicians and interdisciplinary teams


• Educate patients and families to support recovery and long-term outcomes


• Travel throughout the assigned North Side Chicago territory

What You'll Need

• Licensed Physical Therapist in Illinois (or eligible)


• Minimum 1 year of clinical experience (or strong clinical affiliation experience)


• Experience with home-based therapy equipment and assistive devices


• Strong communication, documentation, and time management skills


• Ability to work independently while collaborating with a broader care team
