Job Requirements & Qualifications

Education: Master of Social Work (MSW) required.

Experience: Minimum 1 year; travel experience not required.

Charting: Epic experience required.

Certification: BLS preferred.

Clinical Skills: Biopsychosocial assessments, patient counseling, care coordination, and documentation.

Community Resources: Knowledge of Medicare, Medicaid, Social Security, food assistance, and community-based programs.

Preferred: Oncology experience, hospice care, advance directives, individual/group counseling, and cultural awareness.

Additional Knowledge: Protective services, guardianship, HCBS, and suicide/self-harm risk assessment.
