Physical Therapist (PT) specializing in Skilled Nursing Facility (SNF) care is sought for a location in Los Gatos, California. The position serves an average of 14 to 16 patients per day, supporting patient recovery and mobility goals.

Job Details:

• Location: Los Gatos, CA
• Schedule: Days shift, 8 hours per day, 40 hours per week
• Required Qualifications: Valid physical therapy license, degree in physical therapy, California state licensure, 2 years of experience in SNF or outpatient settings
• Preferred Qualifications: Experience working with senior patients, familiarity with Medicare procedures, certified in manual therapy or orthopedics
• Additional Information: The position involves direct patient care, developing individualized treatment plans, and documenting progress in accordance with facility policies and procedures. Candidates should be adept at working independently and collaborating with a multidisciplinary team.
