Job Details:
- Location: Springfield, IL
- Schedule: Days shift, 8 hours per day, minimum of 40 hours per week
- Required Qualifications: Valid Physical Therapist (PT) license, applicable state licensure, graduate degree in Physical Therapy, minimum of 2 years of experience in home health, and current CPR certification
- Preferred Qualifications: Experience with electronic medical records, familiarity with Medicare/Medicaid regulations, and additional advanced certifications in rehabilitation or therapy
- Float: Float rotation may be required
