Job Summary:
Physical Therapist Outpatient Fort Collins, Colorado. This outpatient therapy position involves evaluating assigned patients, developing individualized treatment plans, and ensuring appropriate progress toward discharge goals. The role emphasizes patient education, interdisciplinary collaboration, and adherence to safety and professional standards to support optimal recovery and functional independence.

Job Details:

• Schedule: Days shift, 8 hours per day, with a minimum of 40 hours per week
• Required Qualifications: Degree in Physical Therapy from an accredited school; 6 months to 1+ year of outpatient clinical experience; CPR certification; active and applicable license for the state of Colorado; successful completion of a previous assignment with the agency or the organization (highly desired)
• Additional Information: Completes comprehensive patient evaluations, creates and modifies treatment plans, and documents medical records accurately and timely. Participates in student education with supervision and provides a positive learning environment. Maintains licensure in good standing and follows all state practice acts and regulatory guidelines. Demonstrates commitment to performance improvement initiatives and organizational safety protocols.
• Float: Float rotation may be required.
