Job Summary
Home Health Physical Therapist Assistant (PTA) - Flexibility and adaptability are essential for this position. This role involves providing outpatient home health therapy services within a designated territory, focusing on geriatric patient populations. The position offers an opportunity to work independently while collaborating with interdisciplinary teams to enhance patient recovery and well-being.

Job Details

• Shift Schedule: Days, 8 hours per day, with a minimum of 40 hours per week.
• Required Qualifications: Valid license in hand, active drivers license and personal transportation, minimum of 1 year of relevant experience, recent six daily visits or at least 30 points weekly for productivity, BLS certification required, proficiency in HCHB (Homecare Homebase) and OASIS for staff completing Start of Care (SOC), experience with outpatient clinics and skilled nursing facilities preferred, ability to work independently with timely documentation and communication.
• Preferred Qualifications: Experience with geriatric patient populations and case management, flexibility to cover territory as needed, previous home health or hospice experience.
• Additional Information: Position involves providing comprehensive outpatient home health therapy to geriatric patients. Candidates should be prepared for occasional travel beyond the territory radius based on patient needs. Orientation begins on Monday, and all travelers must have an active drivers license and personal transportation. RTO (Requested Time Off) must be submitted during application.
• Travel Requirements: Not first-time travelers; travel pairs accepted but each must have own territory and transportation. On-call rotation may be required; discuss during interview.
