Job Summary

Physical Therapist, Home Health. The healthcare provider is seeking a physical therapist to deliver geriatric care in patients homes. The role requires timely documentation, effective communication, flexibility in assigned territories, and the ability to manage approximately 57 daily visits or a minimum of 30 productivity points per week.

Job Details

• Schedule: Day shift, 8 hours per shift, with a minimum of 40 hours per week.
• Required Qualifications:
  • Minimum of 1 year of recent, relevant experience
  • Basic Life Support (BLS) certification
  • Active professional license in hand at submission
  • Active drivers license and personal transportation
  • Proficiency with Homecare Homebase charting system
  • Timely documentation and communication skills
• Preferred Qualifications:
  • Experience completing Start of Care (SOC) visits in home health
  • Experience with OASIS documentation
  • Experience treating geriatric patients
• Additional Information:
  • Approximately 57 patient visits per day or a minimum of 30 productivity points weekly
  • Patient care may include acute care, outpatient clinic, rehabilitation, and skilled nursing settings
  • Clinical experience may include post-operative orthopedic care, neurological conditions, spinal conditions, prosthetics, gait training, fractures, joint replacements, and post-stroke rehabilitation
  • Flexibility with assigned territory is expected
  • Timely documentation and communication are mandatory
  • Scrubs are required; jeans are not permitted
  • A tablet will be provided for clinical use
  • Orientation is scheduled for Monday
  • Requested time off must be submitted at the time of application
  • A home health checklist is required
  • Travel pairs may be accepted, with each traveler assigned independently
