Description
Job Summary
Occupational Therapist, Home Health. This position provides occupational therapy services to geriatric patients in a home health setting. The role requires timely documentation, effective communication, flexibility with territory coverage, and the ability to manage an average daily caseload of approximately 57 patients.
Job Details
- Schedule: Day shift, 8 hours per shift, with a minimum of 40 hours per week.
- Required Qualifications: Minimum of 2 years of recent relevant experience; Occupational Therapist licensure in hand; Basic Life Support (BLS) and NBCOT certification; active drivers license and personal transportation; proficiency with HCHB; experience with required charting systems; OASIS experience for clinicians completing Start of Care visits.
- Additional Information:
- Geriatric patient population.
- Timely documentation and communication are required.
- Territory coverage requires flexibility.
- No first-time travelers accepted.
- On-call rotation may be required and should be discussed during the interview.
- Monday orientation.
- Scrubs are required; jeans are not permitted.
- An iPad is provided.
- Hospice/Home Health checklist completion is mandatory.
- Travel pairs may be accepted, with each clinician assigned an individual territory and responsible for personal transportation.
