Physical Therapist (PT) Home Health in the Mossyrock & Morton Area. This role involves providing patient-centered therapeutic care to geriatric populations, focusing on rehabilitation and functional improvement. The position requires flexibility with territory and adherence to clinical documentation standards.

- Schedule: Days, 8 hours shifts, minimum of 40 hours per week
- Required Qualifications: Valid license in-hand with submission, active drivers license, personal transportation, proficiency in HCHB (Homecare Homebase), OASIS experience required for staff completing Start of Care (SOC) in Home Health, timely communication and documentation mandatory, RTO (Requested Time Off) must be submitted at the time of application
- Additional Information: Experience with acute care, outpatient clinics, rehabilitation hospitals, skilled nursing facilities, and various post-operative conditions is preferred. On-call rotation may be required, and flexible territory assignment is expected. Modules are billable and completed during orientation.
- Submission Requirements: Must have active drivers license and personal transportation, RTO submitted during application, and approval by HR for former employees if applicable.
