Job Details

• Location: Conifer, Colorado
• Position Description: Provides outpatient physical therapy including initial evaluations, safety assessments, and ongoing treatment plans. Duties include developing and modifying therapy programs, delegating services, and ensuring effective patient recovery, with a focus on patient safety and education.
• Certifications, Education, and Experience: Degree in Physical Therapy from an Accredited School, CPR Certification, 12+ months outpatient clinical experience, active applicable state license, and 6 months of outpatient experience desired.
• Schedule: Days shift, 8 hours per day, minimum 40 hours per week.
• Additional Information: Completes comprehensive evaluations, documents medical records accurately and timely, and ensures treatment complies with physician orders and insurance requirements. Participates in student programs, pursues ongoing professional development, and adheres to regulatory standards and organizational performance improvement initiatives.
