Job Details

• Schedule: Dayshift, 8-hour shifts, minimum of 40 hours per week
• Certifications, Education Requirements, Degree, Location, and Experience: Degree in Physical Therapy from an Accredited School; 6 months to 1+ year of outpatient clinical experience; CPR Certification; active applicable state license; minimum of 1 year of experience recommended.
• Responsibilities include completing comprehensive patient evaluations, developing individualized treatment plans, documenting medical records accurately and timely, and ensuring care is provided under signed, valid physician orders and insurance approvals.
• Participates in student programs and provides mentorship, fostering a positive learning environment and supporting professional development through continuing education and involvement in professional organizations.
• Adheres to discipline-specific State Practice Acts and maintains licensure in good standing.
• Promotes a culture of safety by following guidelines for patient and employee safety, including safe patient handling and infectious disease protocols.
• Demonstrates competency in care delivery aligned with organizational performance improvement initiatives.
