• Additional Information: Performs comprehensive patient evaluations, develops treatment plans focused on discharge goals, and collaborates with the interdisciplinary team to address barriers to discharge. Completes medical record documentation in accordance with regulations and organizational policies. Participates in student programs and actively seeks professional growth through continuing education and professional activities. Complies with all regulatory agencies and standards of practice, maintaining licensure and discipline-specific certification. Demonstrates awareness of organizational performance improvement initiatives and contributes to quality improvement efforts.
