Job Summary

Physical Therapist, Home Health West Portland, Oregon and surrounding areas. This role provides one-on-one physical therapy services in patients homes, with visits to up to six patients per day and an expected standard of 30 productivity points. The position includes documentation, care coordination, and home health services across an assigned territory.

Job Details

• Schedule: Day shifts; 8-hour shifts; minimum of 40 hours per week.
• Required Qualifications:
  • Active Oregon physical therapist license in hand at submission
  • Minimum of 1 year of skilled home health experience
  • BCLS certification
  • Active drivers license and personal transportation
  • Timely communication and documentation
• Preferred Qualifications:
  • OASIS experience, required for clinicians completing Start of Care visits
  • Proficiency with Homecare Home Base
• Additional Information:
  • Provides one-on-one patient care in a home health setting.
  • May visit up to six patients per day.
  • Business-casual attire is required; scrubs are permitted but not required.
  • Documentation is completed remotely using Homecare Home Base and mobile applications.
  • Primary COVID-19 vaccination series and one booster are required.
  • Requested time off must be submitted at the time of application.
  • Orientation is scheduled for Monday.
  • A home health checklist is required.
  • Timely completion of orientation modules is required.
  • Weekend coverage, on-call responsibilities, and block scheduling are not routinely required.
