Job Summary

Physical Therapist, Outpatient Charlestown. The Physical Therapist evaluates assigned patients, develops and modifies individualized treatment plans, and provides therapy services to support functional recovery and return to community activities. The role also includes patient and family education, timely clinical documentation, collaboration with physicians, and participation in ongoing professional development.

Job Details

• Schedule: Day shift, 8 hours per shift, 40 hours per week.
• Assignment Dates: 11/09/202602/08/2027.
• Required Qualifications:
  • Degree in Physical Therapy from an accredited school.
  • At least 1 year of outpatient clinical experience.
  • Current Cardiopulmonary Resuscitation (CPR) certification.
  • Active and applicable professional license for the state where services will be performed.
• Additional Information:
  • Complete comprehensive patient evaluations and use clinical reasoning to establish treatment plans and goals.
  • Provide individualized therapy programs designed to promote functional progress and discharge readiness.
  • Maintain clear, accurate, concise, and timely medical record documentation.
  • Provide care under valid physician orders and within applicable clinical approvals.
  • Educate patients, families, students, and coworkers regarding care and therapy services.
  • Participate in student training and supervision when assigned.
  • Maintain professional licensure in good standing and complete required continuing education.
  • Follow applicable state practice requirements, professional standards, safety procedures, confidentiality expectations, and quality improvement initiatives.
