Job Summary

Physical Therapist, Outpatient West St. Paul. The Physical Therapist evaluates patients, develops and implements individualized treatment plans, and provides orthopedic outpatient care. The role also includes patient and family education, clinical documentation, care coordination, and participation in professional development activities.

Job Details

• Required Qualifications: Degree in Physical Therapy from an accredited school; at least 1 year of outpatient clinical experience; current Cardiopulmonary Resuscitation (CPR) certification; professional licensure in good standing.
• Preferred Qualifications: Oncology experience.
• Schedule: Day shift, 8-hour shifts, 40 hours per week.
• Responsibilities: Complete comprehensive patient evaluations; develop and modify treatment plans; establish goals focused on progression toward discharge; provide discipline-specific therapy services; maintain accurate and timely medical record documentation; provide education to patients, families, students, and coworkers; participate in student supervision and professional development; follow applicable state practice acts, regulatory requirements, professional standards, and safety procedures.
