Job Summary

Physical Therapist Home Health. This position provides skilled home health services in a one-to-one patient setting, with approximately 30 productivity points per week and up to six patient visits per day. The assignment is scheduled to begin 11/23/2026 and welcomes clinicians with relevant home health experience.

Job Details

• Schedule: Eight-hour day shifts, 40 hours per week; 4x10 and 3x12 schedules are not available.
• Required Qualifications:
  • At least one year of skilled home health experience
  • Active Oregon Registered Nurse (RN) license required before the start date; temporary licenses are not accepted
  • Basic Life Support (BLS) certification
  • Operating and documenting proficiency with Home Care Home Base (HCHB)
  • Outcome and Assessment Information Set (OASIS) experience
  • Reliable personal transportation and an active drivers license
  • Timely communication and documentation
• Additional Information:
  • Skilled home health setting with a one-to-one patient ratio
  • Approximately 30 productivity points per week and up to six patients per day
  • Weekend support may be required
  • No call requirement is listed
  • Patient visits are grouped when possible to minimize travel
  • Business-casual attire is required; scrubs are permitted but not required
  • Monday orientation is required
  • Requested time off must be submitted at the time of application
  • Home Care Home Base (HCHB), a remote home health client platform, and a mobile phone application are used for documentation and workflow
