Physical Therapist, Home Health Location not specified. This position provides skilled home health services in a one-to-one patient setting, managing approximately six patients per day and an expected productivity level of approximately 30 points per week. The assignment includes eight-hour day shifts and may require weekend support.

Job Details

• Schedule: Eight-hour day shifts, 40 hours per week. Four 10-hour shifts and three 12-hour shifts are not available.
• Required Qualifications:
  • At least 1 year of skilled home health experience
  • Active Oregon Registered Nurse (RN) license before the start date; temporary licenses are not accepted
  • Basic Life Support (BLS) certification
  • OASIS experience
  • Home Care Home Base experience
  • Reliable personal transportation and an active drivers license
  • Timely communication and documentation
• Preferred Qualifications:
  • Previous travel experience
• Additional Information:
  • Skilled home health setting with one-to-one patient care
  • Up to approximately six patient visits per day
  • Weekend support may be required
  • No regular call requirement is listed; an on-call rotation may be discussed during the interview
  • Monday orientation
  • Business casual attire; scrubs are permitted
  • Patient visits are grouped when possible to help minimize driving
