Job Summary
Physical Therapist Home Health Chelsea, East Boston, Revere, and Winthrop. Provides physical therapy services to patients in their places of residence who have physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes. Collaborates with patients, families, physicians, and allied health professionals to identify patient needs and coordinate appropriate services.
Job Details
- Required Qualifications: Physical Therapist professional license in Massachusetts; minimum of 1 year of home care experience; ability to travel to assigned cases.
- Schedule: Day shift, 8 hours per shift, with a minimum of 40 hours per week.
- Additional Information: Provides care to patients ranging in age from birth to over 100 years. Responsibilities include timely electronic documentation, interdisciplinary communication, and participation in weekend rotations approximately once every 6 weeks. Start date is dependent on credentialing clearance.
