Job Summary

Physical Therapist, Home Health position providing skilled home health services in a one-to-one patient setting. The role includes approximately 30 productivity points per week, care for up to six patients per day, and documentation using OASIS and Homecare Home Base (HCHB).

Job Details

• Schedule: 8-hour day shifts, 40 hours per week, with a start date of 11/23/2026. Four 10-hour shifts and three 12-hour shifts are not available.
• Required Qualifications:
  • At least 1 year of skilled home health experience
  • Active Oregon Registered Nurse (RN) license required before the start date; temporary licenses are not accepted
  • Basic Life Support (BLS) certification
  • OASIS experience
  • Homecare Home Base (HCHB) experience
  • Reliable personal transportation and an active drivers license
  • Timely communication and documentation
• Additional Information:
  • Skilled home health assignment with a one-to-one patient ratio
  • Weekend support may be required
  • No call requirement
  • Business-casual attire; scrubs are permitted but not required
  • Monday orientation
  • Requested time off must be submitted at the time of application
  • Patient visits are grouped when possible to support efficient scheduling
  • Home health documentation is completed using a laptop remote client and the Rover phone application
  • Hospice/Home Health Checklist completion is mandatory
