Job Summary

Physical Therapist Assistant, Skilled Nursing Facility, Oklahoma. This role provides skilled, medically necessary rehabilitative treatment to patients with physical functioning disorders under the supervision and direction of a Physical Therapist. Responsibilities include implementing treatment plans, monitoring patient progress, documenting services, communicating with the care team, and supporting patient and caregiver education.

Job Details

• Schedule: Day shifts, 8 hours per shift, with a minimum of 40 hours per week.
• Required Qualifications:
  • At least 1 year of Physical Therapist Assistant experience required.
  • Long-term care or rehabilitation experience required.
  • Active Physical Therapist Assistant license required for Oklahoma.
  • Active occupational therapy license required at the time of submission.
  • New graduates may be considered with qualifying Level II experience beyond observation-only training.
• Additional Information:
  • Provides treatment interventions such as manual techniques, ambulation, therapeutic exercises, modalities, and use of supportive or assistive devices.
  • Assesses patient progress and communicates treatment outcomes, barriers, and recommendations to the supervising Physical Therapist.
  • Provides consultation and education to patients, families, caregivers, and other service providers.
  • Maintains accurate clinical documentation and treatment minutes in the electronic documentation system.
  • Participates in patient care conferences, utilization review meetings, rehabilitation meetings, required training, in-services, and conferences.
  • Supports the selection of necessary durable medical equipment and supplies under the direction of the Physical Therapist.
  • May supervise and support students under the guidance of the Physical Therapist.
  • Assignment period is 11/30/2026 through 03/01/2027.
  • No on-call shifts or weekend shifts are required.
