Job Summary

Physical Therapist, Skilled Nursing Facility Oklahoma. The Physical Therapist evaluates and rehabilitates patients with physical functioning disorders and wound care needs while developing individualized plans of care. The role provides evidence-based treatment, monitors patient progress, coordinates with the care team, and supports patients, families, caregivers, and other service providers throughout the rehabilitation process.

Job Details

• Schedule: Day shifts, 8-hour shifts, with a minimum of 40 hours per week.
• Required Qualifications:
  • One year of Physical Therapist experience required; new graduates may be considered with Level II experience that is not limited to Level I observation.
  • Long-Term Care (LTC) or rehabilitation experience required.
  • Active Oklahoma Physical Therapist license required.
• Additional Information:
  • Perform patient screenings, evaluations, standardized assessments, therapeutic exercises, manual techniques, ambulation training, modalities, and wound care.
  • Use supportive and assistive devices and obtain necessary durable medical equipment (DME) or supplies.
  • Evaluate treatment outcomes, modify services as indicated, and document care in the electronic documentation system.
  • Provide discharge summaries and accurately record treatment minutes in accordance with regulatory and clinical requirements.
  • Communicate patient progress, barriers, and treatment plans with supervisors and members of the healthcare team.
  • Participate in patient care conferences, utilization review meetings, rehabilitation meetings, required trainings, in-services, and conferences.
  • Supervise and support physical therapist assistants, rehabilitation aides, and students.
  • Follow American Physical Therapy Association (APTA) standards, state practice requirements, professional ethics, and applicable codes of conduct.
