Job Summary

Physical Therapist, Outpatient Physical Therapy, Baytown. This position provides outpatient evaluation and treatment services, developing individualized plans of care based on patient needs, goals, and physician direction. The role includes 60-minute evaluations and treatment sessions, patient and family education, clinical documentation, and coordination of discipline-specific therapy services.

Job Details

• Schedule: Day shifts, 8 hours per shift, with a minimum of 40 hours per week.
• Required Qualifications: Degree in Physical Therapy from an accredited school; at least 1 year of outpatient clinical experience; current Cardiopulmonary Resuscitation (CPR) certification.
• Additional Information:
  • Complete comprehensive patient evaluations and develop treatment plans focused on progress toward discharge.
  • Provide individualized therapeutic programs designed to support patients return to community activities.
  • Complete clear, accurate, and timely medical record documentation in accordance with applicable requirements and organizational policies.
  • Provide care under valid physician orders and within applicable clinical approvals.
  • Educate patients, families, students, and coworkers regarding treatment and care.
  • Participate in student education and supervision when assigned.
  • Maintain professional licensure in good standing and complete required continuing education.
  • Follow applicable state practice acts, professional standards, safety procedures, and ethical codes of conduct.
  • Participate in quality improvement initiatives and contribute feedback to support improved patient care.
  • Start date: 11/23/2026. Assignment end date: 02/22/2027.
