Job Summary

Physical Therapist, Home Health Marshfield. The Physical Therapist provides home health services to geriatric patients across an assigned territory. The role includes approximately 57 patient visits per day, timely documentation and communication, and participation in an on-call rotation.

Job Details

• Schedule: Day shift, 8-hour shifts, with a minimum of 40 hours per week.
• Required Qualifications:
  • Minimum of 1 year of recent relevant experience
  • Basic Life Support (BLS) certification
  • Physical Therapist license in hand at submission
  • Active drivers license and personal transportation
  • Outcomes and Assessment Information Set (OASIS) experience required for staff completing Start of Care (SOC) home health visits
  • Homecare Homebase (HCHB) experience required
  • Timely documentation and communication skills
• Preferred Qualifications: First-time travelers are not accepted.
• Additional Information:
  • Provides care for geriatric patients in home health settings
  • Expected patient volume is approximately 57 visits per day
  • On-call coverage is required every third to fourth weekend
  • Flexibility with the assigned territory is required
  • Monday orientation is scheduled
  • Scrubs may be worn in any color; jeans are not permitted
  • An iPad is provided
  • Hospice/Home Health Checklist completion is mandatory
  • Requested time off must be submitted at the time of application
