Physical Therapist Outpatient Colorado SpringsFountain. The Physical Therapist evaluates outpatient patients, develops individualized treatment plans, and provides comprehensive care to support functional recovery and return to community activities. The role includes patient and family education, collaboration with healthcare professionals, and adherence to professional, safety, and regulatory standards.

Job Details

• Schedule: Day shift, 8 hours per shift, 40 hours per week.
• Required Qualifications:
  • Degree in Physical Therapy from an accredited school.
  • 6 months to 1+ year of outpatient clinical experience.
  • Cardiopulmonary resuscitation (CPR) certification.
• Additional Information:
  • Evaluate assigned patients and develop, implement, and modify treatment plans based on patient needs, goals, and physician direction.
  • Provide individualized treatment programs designed to support effective recovery and discharge planning.
  • Delegate and direct discipline-specific therapy services as appropriate.
  • Complete accurate and timely clinical documentation in accordance with applicable guidelines and policies.
  • Provide care under valid physician orders and within applicable clinical authorization requirements.
  • Educate patients, families, students, and colleagues.
  • Participate in student education and supervision when assigned.
  • Maintain professional licensure in good standing and complete required continuing education.
  • Follow applicable state practice requirements, professional standards, ethical guidelines, safety procedures, and infection-control practices.
  • Support quality-improvement initiatives and contribute to a safe patient-care environment.
