Job Summary

Physical Therapist, Outpatient, Houston North Shore. The position provides outpatient physical therapy services, including 60-minute evaluations and treatment sessions. Responsibilities include developing individualized treatment plans, coordinating discipline-specific therapy services, educating patients and families, and supporting safe, effective returns to community activities.

Job Details

• Schedule: Day shift, 8 hours per shift, with a minimum of 40 hours per week. Start date: 11/23/2026.
• Required Qualifications:
  • Degree in Physical Therapy from an accredited school.
  • At least 1 year of outpatient clinical experience.
  • Cardiopulmonary Resuscitation (CPR) certification.
  • Active, applicable license in the state where services are provided.
• Additional Information:
  • Complete comprehensive patient evaluations and develop, implement, and modify treatment plans based on patient needs, goals, and physician direction.
  • Document evaluations, treatment plans, goals, functional quality assessments, medical necessity documentation, and services accurately and promptly.
  • Provide care under valid physician orders and applicable authorization requirements.
  • Educate patients, families, students, and colleagues while supporting assigned students through appropriate supervision and evaluations.
  • Maintain professional licensure, complete required continuing education, and follow applicable state practice requirements and professional standards.
  • Participate in quality improvement initiatives, professional development, and activities that support patient and employee safety.
