Job Summary

Physical Therapist, Skilled Nursing Facility, Texas. The physical therapist assesses movement and functional limitations, develops individualized treatment plans, and provides patient-centered care to reduce pain, restore function, and prevent disability. The role includes ongoing reassessment, patient and caregiver education, interdisciplinary communication, discharge planning, documentation, and participation in quality improvement activities.

Job Details

• Schedule: Day shift, 8 hours per shift, with a minimum of 40 hours per week.
• Required Qualifications:
  • Minimum of 2 years of physical therapy experience in a long-term care setting.
  • Texas or compact license required.
• Additional Information:
  • Conduct patient evaluations by obtaining histories, performing systems reviews, and administering appropriate tests and measures.
  • Establish individualized plans of care and select treatment techniques consistent with physician orders.
  • Reassess patients and monitor their responses to treatment.
  • Initiate and manage discharge planning from the first visit through completion of treatment.
  • Supervise and delegate to ancillary staff and students in accordance with state law and organizational policy.
  • Complete accurate documentation within established timeframes and in accordance with accreditation and insurance requirements.
  • Provide education to patients, families, caregivers, and other members of the care team.
  • Participate in professional development, educational programs, and quality improvement activities.
  • Promote a safe environment for patients, visitors, and staff while practicing within the scope defined by state law.
