Job Summary

Physical Therapist, Outpatient. The position evaluates patients and develops, implements, and modifies individualized treatment plans based on patient needs, goals, and physician direction. The role also provides patient education, coordinates discipline-specific therapy services, supports functional recovery, and promotes safe, high-quality care.

Job Details

• Schedule: Day shifts scheduled Monday, Wednesday, and Friday from 7:00 a.m. to 4:00 p.m. and Tuesday and Thursday from 10:00 a.m. to 7:00 p.m.; 8 hours per shift and 40 hours per week.
• Required Qualifications:
  • Degree in Physical Therapy from an accredited school.
  • At least 1 year of outpatient clinical experience.
  • Current CPR Certification.
  • Active and applicable professional license for the state where services will be provided.
• Additional Information:
  • Complete comprehensive patient evaluations and develop treatment plans and functional goals.
  • Provide care under valid physician orders and within applicable clinical requirements.
  • Maintain clear, accurate, timely medical record documentation in accordance with applicable guidelines and procedures.
  • Provide patient, family, student, and coworker education as appropriate.
  • Participate in student development and supervision when assigned.
  • Maintain professional licensure in good standing and complete required continuing education.
  • Follow applicable state practice laws, regulatory requirements, professional standards, safety procedures, and confidentiality expectations.
  • Contribute to quality improvement initiatives and pursue ongoing professional development.
