Job Overview
This position is a full-time opportunity in the Home Health setting, focusing on providing physical therapy services to patients in their homes. The role involves treating an average of 6.5 patients per day, with a variety of diagnoses including Congestive Heart Failure (CHF), orthopedic conditions, and pneumonia. The therapist will be responsible for initial evaluations, recertifications, and ongoing visits for patients.
Shift Details
- Shift Length: 8 hours
- Guaranteed Hours: 40 hours per week
- Shift Notes: 8 AM to 5 PM, rotating weekends and holidays

Qualifications
- Required Certification: Basic Life Support (BLS)
- Experience: Minimum of 1 year is required; recent graduates are not preferred.
Work Environment
The role offers a supportive environment with a clinical manager or educator available 24/7 for emergency assistance, and from 8 AM to 5 PM for regular support. Orientation includes a two-week training period with a designated preceptor.
Dress Code
Professional attire is required, with scrubs or business casual clothing acceptable.
Additional Notes
Workplace technology, including a phone and tablet, will be provided to facilitate patient care and communication.
