Physical Therapist Home Health
Overview
- Location: Newton, Massachusetts
- Employment type: LTA
- Contract length: 13 weeks
- Shift: 08:00 - 16:30
- Weekend rotations are once every 6 weeks
- Ability to travel to cases as assigned
Required Qualifications
- Valid Physical Therapist license in Massachusetts
- Physical Therapist (1 year of experience)
- Physical Therapist experience within the last year
- Experience in a(n) Home Health setting
- 1 year of case management experience
Job Summary
- Provide physical therapy services to patients in their place of residence
- Patients may range in age from birth to over 100 years
- Collaborate with patient, family, physician, and other allied health professionals
- Use timely electronic documentation and interdisciplinary communications in accordance with agency policies, procedures, and standards of practice
Onboarding / Compliance
- Resume with past 7 years of work experience
- 2 references within 4 years of submission, including at least 1 supervisor or above in skillset
- Skills checklist completed within 1 year of submission
- MVR check required for all home care positions
- Primary source verification of professional license and/or certification
- N95 fit test within 365 days of start date
- Respirator Medical Clearance attestation if fit test is required for the skill
- Nurse Aide Registry check
- MMR documentation or proof of immunity
- Influenza vaccine
- TB test documentation
- Health statement signed by MD/DO/NP/PA
- Criminal background check
- Massachusetts CORI
- SSN trace
- National Sex Offender Check
- Exclusion checks
- Education verification
- Confirmation of Assignment/POI form
- Beeline attestation
