Required Qualifications
- Valid Pharmacist license in Oklahoma
- Pharmacist (2 years of experience)
- Pharmacist experience within the last year
- BLS
- Driver's License
Required Skills / Experience
- Review patient records to determine appropriateness of medication therapy
- Evaluate patient condition to ensure issues are being treated
- Develop medication plans that minimize risk of adverse side effects
- Consult with prescribers, nurses, and patients on medications and doses
- Advise on correct administration of drugs
- Assess results and outcomes of pharmaceutical treatments
- Collaborate with healthcare professionals to ensure optimal patient care
- Keep accurate documentation of medication plans and patient progress
- Routinely consult with physicians and nurses regarding orders, medication availability, side effects, incompatibilities, formulary considerations, and current care path management
- Participate in education of students and residents as needed
