Pharmacist Outpatient

Overview

Location: Fort Worth, Texas
Employment type: Travel/Contract
Schedule: Days, Monday–Friday, 8:00–17:00, with 1 hour lunch
Hours: 40 per week
Onsite role seeing 14 patients per day

Required Qualifications

Doctorate of Pharmacy or equivalent from an accredited College of Pharmacy
2-year ASHP residency in HIV
5 years of direct HIV patient care demonstrating medication management
Texas State Board of Pharmacy license or eligibility for the same
BLS

Role Summary

Clinical Pharmacist Disease Specialist for Amb HIV Hep C
Provides clinical leadership for pharmaceutical care
Serves as a resource for outpatient and inpatient clinicians for HIV and/or Hepatitis pharmacotherapy
Works with the Inpatient Infectious Diseases Clinical Pharmacist to transition HIV patients from the hospital setting to the HIV Clinic on appropriate therapy
Develops and optimizes pharmacotherapeutic regimens through collaborative practice agreements
Performs drug utilization reviews and monitors for drug interactions and potential adverse events
Works to minimize polypharmacy and develops patient medication action plans with self-management goals
Precepts pharmacy students or residents and participates in didactic and experiential education
Participates as a key component of the multidisciplinary patient care team in the Medical Home

Pros

Marigold is polite and professional. She goes above and beyond to ensure the contractor is well informed and has everything needed to be successful. She helped me with onboarding and getting used to the timesheet app and she answers my texts promptly, even after hours. She is a great recruiter liason. Gladys was a super helpful with onboarding and Denise will hook you up with benefits with no trouble at all. Great team to work with.

Cons

Onboarding is somewhat tedious, but they do dot every "i" and cross every T.
