Assignment Overview

• Location: Madras, Oregon
• Float coverage: 3 shifts per week in Madras and 1 shift per week at another facility location (Bend, Redmond, or Prineville), depending on where need is greatest. Clinicians will float within scope to meet facility needs, including Prineville.
• Contract length: 13 weeks
• Schedule: Night shifts, 4x12-hour shifts (17:45 to 6:15), 48 hours per week
• Weekend requirement: Every other weekend
• Patient population: Some pediatric patients (from the bilirubin/bili baby program) and some IMU-level patients. Ratios are acuity-based, and most patients are low acuity.
• Time off: Maximum of 7 total days off during the 13-week assignment

Required Qualifications

• Valid RN license in Oregon
• Med Surg / Telemetry experience (2 years)
• Med Surg / Telemetry experience within the last year
• Experience in a hospital setting
• Associate of Science in Nursing (ASN) / Associate Degree in Nursing (ADN)
• BLS certification issued by the American Heart Association
• NIHSS certification
• Experience with Epic

Eligibility Restrictions

• Accepts first-time travelers
• Does not accept local candidates (75-mile radius rule). Submissions require proof of address.
• Does not accept pending licenses

Onboarding Requirements

Background and verification

• 7-year criminal background check within 90 days of start, updated annually. Includes SSN trace, counties lived/worked/schooled, FACIS L3, DOJ sex offender search (nationwide), and highest-level education verification.
• EPLS/GSA/SAM and HHS/OIG checks within 90 days of start
• Diploma or transcript on file for all positions, plus education verification

Health requirements

• Physical exam within 1 year of start (not required annually)
• Tdap within 10 years of start, given after age 19 (declination accepted)
• TB: negative QuantiFERON-Gold within 1 year of start. Positive responders need a negative chest X-ray within 5 years and annual TB questionnaire. TSpot and PPD are no longer accepted as of 4/2026.
• TB questionnaire required annually
• Measles (rubeola), mumps, rubella, and varicella: positive titer or 2 vaccines. Low or equivocal titers may be resolved with 1 booster or declination.
• Hepatitis B: positive titer required (vaccine series alone is not acceptable). Low or equivocal titers may be resolved with a booster or declination.
• Negative 12-panel drug screen within 90 days of start (not required annually)
• Flu vaccine or declination/exemption required during flu season (10/1 to 3/31). Vaccine is preferred, and masking is required during high transmission. Deadline: 12/01.
• COVID vaccines are preferred but not required. Declination or exemption is required if never vaccinated.
• RespSafety clearance within 1 year of start and annually
• Skills checklist required annually

Facility documents

• Passport-style photo with solid background for badge
• Acceptable use and confidentiality agreement, contractual and third-party access agreement, compliance and privacy attestation, and emergency contact form
• Facility documents must be on file within 48 hours of signing and no later than 2 Wednesdays before start.
