Description
As a case manager RN, you will primarily work Monday through Friday with a rotating weekend every third week. Your role involves managing a case load of 12-15 patients, performing routine visits, recertifications, and admissions. You will collaborate closely with patients and their families to provide comprehensive hospice care, ensuring their comfort and dignity.
