Description
In this role, you will provide compassionate hospice care to patients in their homes. Your schedule will rotate between day shifts for home visits and evening shifts for phone support. Expect to manage a caseload of multiple patients, ensuring their comfort and communication with the care team. This position emphasizes a team-oriented approach and requires strong organizational skills to handle case management effectively.
