Description
As a Home Health Physical Therapist, you'll work with patients in their homes, providing evaluations and creating tailored treatment plans. Your role includes delivering skilled interventions, monitoring progress, and educating patients and families on safety and rehabilitation techniques. You'll collaborate closely with healthcare professionals to ensure optimal patient outcomes.
