Position Overview

We are seeking an experienced Registered Nurse – Home Health to provide skilled nursing care to adult and geriatric patients in their homes. The ideal candidate will have recent skilled home care experience, strong assessment and wound-care skills, and experience managing medically complex patients.

Private-duty nursing experience alone will not be accepted.

Requirements

• Minimum 1 year of skilled home health experience
• Recent patient-care experience within the last 3 months
• Previous OASIS training and experience required
• Must have completed more than one travel assignment
• BLS/BCLS required
• Valid driver's license and reliable vehicle required for home visits
• Must be comfortable completing approximately 6–8 home visits per day
• Strong IV and wound-care experience required
• COVID vaccination or approved exemption required
• Flu vaccination or declination required; mandatory mask wearing applies at work

Required Clinical Skills

Candidates should have current hands-on experience with:

• Cardiopulmonary assessments
• Wound care
• ABI compression dressings
• Wound VAC dressing changes
• PleurX/Asept drain care
• Foley catheter care and changes
• Staple and suture removal
• Glucometer use
• Pro-time machine
• Venipuncture and lab draws
• IV therapy
• PICC line and Midline care/dressing changes
• Subcutaneous port access/deaccess
• SQ and IM injections
• Medication administration and management

Patient Population

Patients include adults and geriatric patients with:

• General medical diagnoses
• Heart failure (HF)
• COPD
• Myocardial infarction (MI)
• Pneumonia
• Stroke
• Post-operative conditions
• Other medically complex conditions requiring skilled home care

Schedule

The home health office may offer varying schedules based on operational needs:

• Monday–Friday: 8:00 AM–5:00 PM
• Monday–Friday: 9:00 AM–6:00 PM
• Thursday–Monday
• Friday–Tuesday
• Alternating weekend shifts

Visits and working hours are generally self-scheduled based on the needs of the HomeCare office.

Workload & Orientation

• Approximately 6–8 skilled home visits per day
• Patient ratio: Varies based on daily assignments
• Weekend coverage: Alternating weekends
• No pre-approved time off
• Patient care is provided through Medicare, Medicare Advantage, and Commercial Insurance programs

Important Candidate Requirements

This position requires true skilled home health experience with OASIS, not private-duty nursing alone. Candidates must be comfortable independently managing multiple home visits per day and performing skilled assessments, wound care, IV therapy, and other home health procedures.
 

A valid driver's license and personal vehicle are required for making patient home visits.
