RN Case Manager

Eureka, CA

Start Date: November 16, 2026
Contract: 13 Weeks
Shift: Days | 5x8s | 8:00 AM – 4:30 PM

Charting: Epic
Weekend Requirement: Every other weekend

Call: Not required

Required Qualifications:

Minimum 2 years of acute hospital Case Management experience.
Recent acute-care hospital Case Management experience within the last year is a hard requirement.
Experience in care coordination and discharge planning.
Experience with HMOs, IPAs, and related healthcare organizations.
Experience with advance directives, POLST, and protective services processes.
Epic experience required.
Travel experience preferred; first-time travelers may be screened with clinical approval.
Must be willing to float as needed.
Must be available to work every other weekend.

Admission criteria, appeals, and denials
Benefits eligibility and prior authorizations
DME needs assessment and coordination
CMS, DRG, HIPAA, and Joint Commission requirements
Experience across ED, ICU, Medical-Surgical, Pediatrics, and Stepdown/PCU/Observation settings
